I like when a hypnotic effect in the background also hides the subject's eyes. Eyes are windows to the soul, after all. I'm also a big fan of works where the background is overlaid with text, like post 'id: 143767' by c120run and post 'id: 136220' by edensnake feels like a nice way to spice up a manip.
