Well obviously her eye color is because of the light from the doll’s eyes and she’s dressed up to fit in with the dolls.

a nearly 100% artificial being made by xemnas.
She 'looks like kairi' because some characters see her like that, roxas, axel. to others her looks change based on what you see her as, xemnas and saix see a faceless doll, xigbar sees ventus. Until three where once she meets sora and the heart that she developed over her story enters her new body and she becomes a kairecolor full time.

I don't think she died and made a heartless in this pic.. I think they just possessed her normal body, and gave her the outfit. cause a lot of enemies in the game are actual toys that heartless take over... besides to my knowldge no one but Xehanort has a heartless that resembles a normal (by kh standards) human being.
