I generally agree with this sentiment. Even having looked through the pictures tagged with nightmare_fuel, it doesn't really seem there is a clear, objective definition of nightmare_fuel. (Which is why I personally don't use it.) I've seen a lot of people use the tag on things they find creepy, and then the tag later gets removed.
If I had to provide a solution to replace the tag, here's what I'd do:
- Remove the tag
- Add a creepy_face tag (would apply to deformed, distorted, or lifeless faces and would cover 75% or more of the nightmare_fuel images)
- Use the parasite tag more liberally (any time someone has some foreign life form coming out of their mouth or genitals)
- Use the insect tag more consistently, where appropriate
- Use the brain_injection tag where appropriate
- Add tags for brain eating and possibly something dealing with implied or imminent death.
The problem seems to be that the nightmare_fuel tag is being used to cover all of these different situations, so it's not really clear what it's purpose is.

I feel like you wouldn't need to do creepy_face(still feels subjective) as much as distorted_anatomy(vs exaggerated_anatomy when someone has ahegao, or unreasonably large breats but not in a terrifying way). This would also cover body parts that are arranged in such a way to be disturbing including face, though you could have both I suppose.
As for brain-eating, I mostly see the nightmare_fuel in that case added to pictures that have pseudo zombie like qualities. (The ones where mushrooms are growing out of obviously deceased if not soon to be deceased people). And just be more liberal with a zombie tag or something similar for those implied to be killed during the hypnosis process.
