They're hermaphroditic, and reproduce by spawning a mass of larvae that are left to develop in the pool of the Elder Brain.
After about a decade, the ones that survive are inserted into the brains of sapient creatures. They grow and eventually consume their host's brain, absorbing their physical form and becoming sapient themselves in a process called <<forgottenrealms.fandom.com/wiki/Ceremorphosis|ceremorphosis>>.
