You know I always grappled with that too. Especially with all of her interpretations.
Guessing it's a combo of her body type, loving swimming, and least in the anime her being headstrong.

She's not a beauty, but her physique is definitely girly. She's not demure, but her feisty personality is especially girly. She doesn't wear a dress, but her clothes are very girly. And she's relatively sporty, but swimming is the girliest sport. Everything about her is girly, just not your regular "beauty". Is that all there is to being a tomboy?

I've never understood why people think 'tomboy' is considered contrary to 'feminine' myself. Looking at the origins of the word, words like 'brash', 'boisterous' or 'immodest' come up, but nothing about 'unfeminine' per se. Hell, one early synonym for the word is 'strumpet', which for those unaware is another word for a "loose woman". Maybe it's just me, but a woman known for carnal relations with men doesn't strike me as 'unfeminine', no matter the perceived moral virtue or lack thereof of such behavior.
But enough of the gutter, let's return to tomboys. It's been well over a decade since I watched the anime, but I seem to recall Misty consistently being short-tempered, loud and often having quite scathing things to say about others to their face, so 'immodest' or 'brash' certainly are words that spring to mind on the personality front. As someone who often places greater weight on initial meaning and thus the 'spirit' of a word over modern 'letter' (especially for a word related to gender roles and other such heated topics), I for one am more than happy to consider Misty a tomboy.

Also, bear in mind that Pokemon originates from Japan, a fairly conservative country where the concept of "yamato nadeshiko" is pretty much the gold standard for femininity, characterized by traits like decorum, kindness, humility, grace or patience. Doesn't exactly describe Misty, I trust we can all agree. Point being, even if you insist that 'tomboy' has something to do with femininity, in the original Japanese context it's a markedly fairer assessment to call her one.
