Because Chihiro only dresses like a girl to avoid people picking on him for how girly he looks.

At least to my understanding, any body issues he had were due to external factors, not internal ones.

Chihiro calling his own body degenerate implies that on some level he has internalized the toxicity of his bullies.

And I do not think that can be called healthy on a psychological level.

I don't remember it that way! I remember Chihiro being a completely confident girl lover who couldn't wait to be one too!

(Or to drop kayfabe a bit, this is Chihiro with the personality of Tenko, and thus a very different view on their body, I support both Cis and Trans readings of Chihiro as being valid and they're an amazing character either way :) )

With this whole thing being about characters getting their personalities swapped, having Tenko's would fundamentally change how Chihiro sees himself.
However isn't that serious, it's not some divine statement, it's a fetish.
