the other girl is likely a genderbent sawada tsunayoshi. the KHR doujin base seems to have a liking for making him female, as well as pairing him with one or more of his 6 guardians (usually hibari or mukuro)

i'm pretty sure. its a fairly common theme in KHR doujin. he may just be crossdressing for all i know, but i'd say theres a fair chance either way
