What do they mean by second hand smoke?
Its when you breath the smoke of someone else, for example like those movies when a boy kisses a girl when he has smoke in his mouth an transfers it to her mouth, or just breathing the smoke of some random person on the street

Well, second-hand smoke is literally that- You're breathing in smoke exhaled from someone else who's actually smoking.

Also called "Passive Smoking" in some countries.

I've always heard it as the smoke coming off the other end of the cigarette. That's actually more dangerous (in some ways) than regular smoking, but I don't think there's any reason why breathing exhaled smoke would be more dangerous... unless that person has a contagious disease :P
It's due largely in part to the fact that the person taking in the second hand smoke is breathing in more harmful particles to some degree. The person with a cigarette has a filter at least; the smoke that they're exhaling doesn't come with that. Darker than Black had a character that said at least twice the amount of particles of second-hand compared to actually smoking (granted my googling couldn't find anything definitive and it's an anime so not necessarily factual, but it's not exactly a light-hearted anime and at least a magician was forced to say how tricks worked, so there might be some truth).
