The better question is what the fuck's wrong with Ash in the first panel? He has one circular eye and one almond-shaped one, his nose is halfway across the left side of his face (his left), his teeth fuse together on the left, and his hand (the one we CAN see) doesn't appear anatomically correct, whereas Ash is perfectly fine anatomically once they've been genderbent by Ghastly the Hypnotic Ghost.

eye twitching? also, drugs, and possibly a fractured mind since he cannot fathom the fact that he'll never be a pokemon master since Red is superior to him in every way.
