In Fate, Grails are wish granting devices. The lady, who is an artificial human, is given over to one as a sacrifice to help it manifest in the physical world. This is part of what she sees inside of it.
This particular grail is infested with the Zoroastrian god of evil. Any wish it grants is going to be bent in order to help bring its influence into the world. Part of what grail can do is tell what your wishes are(as long as you can imagine it). It uses this ability to play on her fears. The pile o' realdolls representing afear of her and her daughter being treated like yet another mass-produced and disposable drone like the rest of her kind.
As the Grail swallows her up, it grants her wish of preserving what little identity she could scrape together over her short life, but in the same move, she is turned into a representative for the grail, and a vector for its corruption.
The music playing the background is a slower, dissonant version of her character theme - I always found that a nice touch.
