In the game he is referred to with male pronouns, and it’s the same in the Japanese translation. Although, napstablook is referred to with gender neutral pronouns in the fangamer version of the artbook. So as far as I’m concerned napstablook is a boy.

Not trying to start an argument, but I'm fairly certain the game exclusively uses "they" for Blooky and the other ghosts.
