More accurately, in Lore Clownpiece is considered an enemy to the Lunarians since she is basically a walking beacon of impurity and is with the main 'villains' (Putting it like that since the Lunarians aren't much better, the only reason why the heroes end up helping out is because of the fact that they were gonna destroy Genskcyo otherwise) Junko and Hectica (latter of whom is her master). Also in lore she remembered the American flag which pissed the Lunarians off, so in a based move made her outfit just like that.

Outside of lore, Lunar Captial is based on China (no not Hong Meiling) and ZUN felt the biggest enemies to them was the USA which is why Clownpiece has that design.

Also ZUN making a fairy (who in touhou are really dumb) from HELL who drives others to insanity wear USA colours is funny. God I love Touhou.
