Power of Chaos is one thing, but when did she fuse with Kefka?
She didn't. The clothes are actually based off her outfit in Dissidia, which in turn is based off Amano's artwork instead of her actual in-game appearance- which is why you'll see some images of her as a blond- although Amano himself was the one who suggested changing her hair to green because looking back at it, he decided he didn't like the idea of all three female PCs having the same color hair. If you're talking about the color scheme of Terra herself, I'll quote the artist on it:
"Initially, I tried to make her look more like Chaos, but it just didn’t feel right, so I went with a succubus-like form with a color scheme similar to Terra’s esper form."
