I'm honestly not very fond of the Kansen OAVs (they take too many liberties from the source), but the ones for the third game were decent. They had better production values and at least sort of followed the game's story.

This is loosely based on a bad ending where Ren gets infected and goes yandere after the main character runs off with his remaining friends, who all happen to be girls. Ren interprets this as "they're trying to steal him away from me" and goes nuts.
She didn't actually kill anyone in the game's bad end, instead she eventually catches up to him, knocks him to the floor, and has her way with him. In the OAV, the protagonist hooks up with another girl (Margarita) but at the very end of the episode, Ren pops out of nowhere and kills her.

I guess that ending stood out because it was shocking to see Ren, who was normally soft spoken and shy, flip out like that. Also, Ren was fairly flat chested in the game, the anime gave her a boob job.

I think taking liberties from the source material (in a bad way) is one of the biggest problems with hentai anime. So many games or h-manga I really liked getting an almost unrecognizable adaptation that oftens misses the point of what was the main draw of the source in the first place.
I don't get why one would watch a lackluster or outright bad adaptation instead of just playing or reading the source material, but somehow the adaptations always end up way more well known.

Been years since I saw the FIRST 3 Kansen Hentai. Never knew they were a game to begin with. And maybe it's nostalgia for me, but I loved them. Loved the ending song to the 3rd OAV, still have it saved. But I also found a horror movie called Kansen and it was pretty good
