This pic honestly scares the crap out of me. I do love seeing whatever comes from the Madoka fandom when it comes to fanart, though.

AFAIK this is based on the Madoka PSP game. Most characters have a chance of turning into a witch in the different storylines if you screw up. This design of Mami seems to be heavily inspired by Mami's witch version in the game.

actually Mami's witch is Candeloro which is more of an actual doll like one not the Charlotte inspired one here.
