Because the brain stuff is part of the fetish. You wouldn't go to a bondage pic and go "But why does they need to be tied up? Why must someone being groped always be tied up first?"; it's a picture of someone being tied up and played with because people find that hot. Similarly, the mental degradation here is something some people find hot, including myself.
And uh, complaining that the brain is "always" destroyed or shrunken is a way overstatement. There's plenty of MC pics that don't involve brain drain/mindbreak on here.
