Onii-chan does mean "brother", but my personal headcanon for the scene paints her more like "that one best friend who is so close she might as well be his sister", though that might be my squick at incest talking. Still, barring the incest overtones, this is one of my favorite text manips to date.

In the series that she's from she was adopted at a very young age by her new family after her parents died. Sorta incest if you count adopted family members.
