If you use Greasemonkey or a similar browser script add-on, this recently-updated userscript supposedly adds a download button to twitter for the full-res version of images:

github.com/Sibusten/twitter-image-downloader

I haven't tried it, yet, though. So grain of salt and all.

Edit: It works. "Download" text-link appears to the right of the date on the little overlay that pops up when you mouse over an image in a twitter image gallery.
